Your First Line of Contact
Our patient advocates are your first line of contact when you reach out to ConnectCare3. They will work with you to understand your needs and get you enrolled in the appropriate service.
Under the direct guidance and supervision of a registered nurse, our patient advocates assist our clinical team conduct research.
Additionally, they can assist callers to identify treatment options for mental health concerns.
Patients with complex health conditions may require both medical and social service support. The patient advocates have extensive experience assisting the registered nurses in identifying programs to meet the needs of our patients.
