Title: Nurse Navigator

Primary responsibility:

My primary responsibility as a nurse navigator is to ensure my patients are educated about their diagnosis and have the necessary resources to make well-informed decisions about their health care. Navigating the healthcare system is a complex, ever-changing process and I am here to support my patients through their journey with a common goal of getting the best possible outcomes..

Why I love my job:

Helping patients unlock the healthcare system and empowering them to be their own best advocate is extremely rewarding. I take great pride in being a valuable resource to both my patients and their families alike.  Moreover, the nurse navigator role perfectly combines my favorite aspects of nursing: patient education, advocacy, problem-solving and research.  The wide variety of diagnoses we encounter creates an opportunity for learning every day and helps to keep us up-to-date on current healthcare issues, treatment options, medications, etc.

How ConnectCare3 helps:

ConnectCare3 is such a unique service in that we are able to personalize care in a way that is difficult to match. As nurse navigators, we recognize the uncertainty that comes along with a new diagnosis so we aim to address each individual’s concerns and potential barriers to care in order to help formulate an attainable plan. Each and every patient’s goal of care may be different and ConnectCare3 helps to individualize every patient’s journey through the system.

Clinical experience:

I graduated from Alvernia University in Reading, PA with my Bachelor’s degree in Nursing. I have nearly ten years of clinical experience in a variety of settings including the emergency department, hematology and oncology, and organ transplant.
